The road was closed in both directions between Wood Street and Murray Road, and motorists were urged to avoid the area where possible.<br /><br />The balloon landed safely at Yarra Bend Park, police said. One of the two was supervising the work on SQ256 – the other engineer was a new hire – while also assisting with turnaround of another aircraft.<br /><br />Covers were fitted to all four pitot probes at Brisbane in line with company procedures. The supervising engineer entered the cockpit to record this cover-fitting in the technical log and place a warning placard on the engine controls. When the aircraft’s first officer conducted a pre-flight walk-round, about 1h later, he probably saw that the pitot covers were in place, as required, says the Australian Transport Safety Bureau.<br /><br />After attending to another aircraft, the supervising engineer returned to SQ256 to discuss fuel figures with the colleague and the crew, before re-entering the cockpit, removing the placard and clearing the log entry for the pitot-cover fitting.<br /><br />According to the inquiry, this was done “without visual or verbal confirmation” that the pitot covers had actually been removed. The supervisor and the colleague then spoke on the ground, near the A350’s nose, about attending to another aircraft. The colleague then remained with SQ256 to conduct pushback headset duties. But around this time, says the inquiry, an aircraft refueller working on an adjacent bay noticed that SQ256 appeared ready for pushback but that its pitot covers were still fitted.<br /><br />The refueller immediately pointed to the A350, informing the supervisor that the covers remained attached. SQ256’s crew had requested pushback from air traffic control, and turned on the aircraft’s beacons, around the same time as the supervisor returned to the aircraft to alert the colleague. Investigators examining the SQ256 incident found that Heston MRO, despite the earlier A330 incident, had “not yet implemented an acceptable method” to account for tools and equipment prior to pushback.<br /><br />The inquiry also draws attention to the possible risks of fatigue, noting that Heston MRO did not track work hours of personnel with dual roles. It states that the supervising engineer had also been serving as Brisbane regional manager, and the demands of these posts had become more demanding in 2022 as aircraft were removed from storage post-pandemic and airline turnarounds increased.<br /><br />Heston MRO no longer requires the Brisbane regional manager to hold dual responsibilities and has started tracking employees’ work hours for fatigue management. Singapore Airlines has provided Heston MRO with pitot covers featuring longer streamers, to make them more visible. The airline's JQ739 flight from Melbourne (MEL/YMML) to Launceston (LST/YMLT) on Saturday was reportedly 'barreling down the runway' when the pilot aborted the takeoff and brought the plane to an abrupt halt.<br /><br />It's understood the door on a small communications panel on the underside of the plane near the wheels had been left open. Ground crew use the communication panel to connect their headsets. <br /><br />'We were just barreling down the runway in Melbourne for takeoff and then the pilot literally slammed on the brakes because apparently air traffic control told them a door was open on the plane when they were trying to take off,' one passenger told reporters.<br /><br />Another passenger said the pilot was warned about the latch by air traffic control seconds before they were about to takeoff. 'We were going for it on the runway, but we had to make an emergency stop. They slammed the brakes on. <div>The aircraft originally started its take off roll at 2.46 pm local time but had to abort. The aircraft taxied back to the gate and was on the ground for forty minutes. It said no emergency had been declared because there was no indication of the damage during flight.<div>United said it would also start its own investigation. <br /><br />“<i>We’ll conduct a thorough examination of the plane and perform all the needed repairs before it returns to service,</i>” the airline said.<br /><br />Boeing did not comment, directing questions to United Airlines.<br /><br />US flight incidents are getting more attention after a door plug blew off an Alaska Airlines Boeing 737 MAX 9 in mid-air. Boeing has been under heavy regulatory scrutiny since the 5th of January incident, with investigations under way into the company’s safety and quality standards in its production process.<br /><br />Last week a United Airlines-operated Boeing 737 MAX rolled off the runway in Houston, prompting investigations; and a United-operated Boeing 777-200 bound for Japan lost a tyre after takeoff from San Francisco and was diverted to Los Angeles where it landed safely.<br /><br />A United Boeing 737 bound for Florida from Houston on the 4th of March returned to the airport shortly after takeoff because the engine had taken in plastic bubble wrap that was on the airfield before departure. According to an affidavit filed by a Federal Air Marshal, 19-year-old Nathan Jones got out of his seat, 6A, and tried to open the cockpit door three separate times during a flight on the 3rd of March, which was travelling from San Diego (SAN/KSAN) to Dulles International Airport (IAD/KIAD) in Virginia.</div><br />On the third attempt, flight attendants “requested the assistance of off-duty law enforcement officers, who restrained Jones in flex cuffs and sat on either side of him for the remainder of the flight,” Pattinson reported.<br /><br />The cockpit door was blocked by a beverage cart and a flight attendant stood guard until they safely landed. Jones, a student pilot, told flight attendants he was “testing them” when asked to explain his dangerous actions, according to the affidavit.<br /><br />He was arrested and has since been charged with alleged interference with a flight crew, which carries a maximum penalty of 20 years behind bars. More than 10 St John vehicles rushed to the airport after they were notified at 3.58pm on Monday of an incident onboard Chilean airline LATAM.<br /><br />St John crews treated approximately 50 patients, with one patient in a serious condition and the remainder in a moderate to minor condition. 13 patients had to be transported to Middlemore Hospital by ambulance.<br /><br />The aircraft in question was Latam Airlines flight LA800 from Sydney (SYD/YSSY) to Auckland (AKL/NZAA), which encountered a “technical problem” that caused strong movement.<br /><br />According to passengers, the plane just dropped. People were thrown out of their seats, hit the top of the roof of the plane, and people throw down the aisles." Another passenger said there were even "blood coming from several people's head.<br /><br />“The plane landed at Auckland Airport as scheduled. As a result of the incident, some passengers and cabin crew were affected. They received immediate assistance and were evaluated or treated by medical staff at the airport as needed,” an airline spokesperson told the Herald.<br /><br />“Latam regrets the inconvenience and injury this situation may have caused its passengers and reiterates its commitment to safety as a priority within the framework of its operational standards.”<br /><br />St John sent four ambulances, two operations managers, one major incident support team vehicle, one command unit and two rapid response vehicles to the scene. One woman onboard the flight said she experienced a “quick little drop” during the flight.<br /><br />The passenger said she saw “at least” five people being taken away by waiting paramedics after landing and was pleased to have made it safely to New Zealand. There were one hundred and forty-nine passengers and eight crew.</p><div>Ethiopian Airlines Flight 302 was a scheduled international passenger flight from Addis Ababa-Bole Airport (ADD/HAAB), Ethiopia to Nairobi-Jomo Kenyatta International Airport (NBO/HKJK), Kenya<br /><br /><ul><li>One minute into the flight, the first officer, acting on the instructions of the captain, reported a "flight control" problem to the control tower.</li></ul></div><div><ul><li>Two minutes into the flight, the plane's MCAS (<b>Maneuvering Characteristics Augmentation System</b>) activated, pitching the plane into a dive toward the ground. The pilots struggled to control it and managed to prevent the nose from diving further, but the plane continued to lose altitude.</li></ul></div><div><ul><li>Three minutes into the flight, with the aircraft continuing to lose altitude and accelerating beyond its safety limits, the captain instructed the first officer to request permission from air traffic control to return to the airport. Permission was granted, and the air traffic controllers diverted other approaching flights. Following instructions from air traffic control, they turned the aircraft to the east, and it rolled to the right. The right wing came to point down as the turn steepened.</li></ul><ul><li>Five minutes into the flight, the pilots struggled to keep the plane's nose from diving further by manually pulling the yoke, the captain asked the first officer to help him, and turned the electrical trim tab system back on in the hope that it would allow him to put the stabilizer back into neutral trim. However, in turning the trim system back on, he also reactivated the MCAS system, which pushed the nose further down. The captain and first officer attempted to raise the nose by manually pulling their yokes, but the aircraft continued to plunge toward the ground.</li></ul><ul><li>Six minutes later the aircraft disappeared from radar screens and crashed at 08:44.</li></ul><br />This was the second MAX 8 accident in less than five months after the crash of Lion Air Flight 610 on the 29th of October 2018. The incident resulted in a series of navigation errors, but the Airbus A320's 153 passengers and four flight attendants were unharmed during the two-hour-and-35-minute flight.<br /><br />About half an hour after the plane took off, the captain asked permission from his second-in-command to rest for a while, with the request being granted. The co-pilot then took over command of the aircraft, but also inadvertently fell asleep himself, the report said.<br /><br />"The second-in-command had one-month old twin babies. His wife took care of the babies, and he assisted while at home," the report said. A few minutes after the last recorded transmission by the co-pilot, the area control centre in Jakarta tried to contact the aircraft several times. It received no answer.<br /><br />Twenty-eight minutes after the last recorded transmission, the pilot woke up and realised his co-pilot was asleep and that the aircraft was not on the correct flight path. He immediately woke his colleague up, responded to the calls from Jakarta and corrected the flight path, the report said.<br /><br />Investigators did not identify the pilots, but said they were both Indonesians and were aged 32 and 28. Investigators thought the most likely location for the jet was in the Indian Ocean after analysing information from the British satellite telecommunications company Immarsat. Likely locations for the airliner could be tracked by knowing the distance from the fixed satellite, but it would also change depending which direction the plane was flying in after its last known position and at what speed it was travelling at. If it was flying north then possible locations could stretch as far as the border between Kazakhstan and Turkmenistan to Thailand. </div><div><br /></div><div>But if it was flying south possible sites could range from Indonesia to the southern Indian Ocean. Authorities believe the Indian Ocean is the most likely site After 7 years of extensive searches and many possible sightings of floating objects, Investigators have found no trace of the Malaysia Airlines 777 or it's 239 passengers.</div><div><br />Initially search efforts focused on the South China Sea area. On the 24 March 2014 further analysis of the Inmarsat satellite data indicated that MH370 flew south and ended its flight in the southern part of the Indian Ocean. A surface search was conducted of probable impact areas along an arc, identified by calculations based on Inmarsat data. The search was carried out from the 18th of March - 28th April 2014. </div><div><br /></div><div>This search effort was undertaken by an international fleet of aircraft and ships with the search areas over this time progressing generally from an initial southwest location along the arc in a north-easterly direction. No debris associated with MH370 was identified either from the surface search, acoustic search or from the ocean floor search in the vicinity of acoustic detection, which were initially believed to have been from the pingers on the flight recorders. The ocean floor search was completed on the 28th of May 2014.</div><div><br />On the 26th of June 2014 the ATSB published a new search area based on refinements to the analysis of both the flight and satellite data. The priority area of approximately 60,000 km2 extends along the arc for 650 km in a northeast direction from Broken Ridge, an underwater ridge. The width of the priority search area is 93 km.<br />On the 29th of July 2015 a flapperon washed ashore on the French island of Réunion in the Indian Ocean. On the 5th of August it was established to have been from MH370.<br /><br />Flight 370 was operated by a nearly 12-year-old Boeing 777-2H6ER, registration 9M-MRO (MSN 28420). This was the 404th Boeing 777 produced, it first flew on 14th May 2002 and was delivered to Malaysia Airlines on 31st May 2002. However, even though the plane already landed, passengers onboard were not able to disembark until approximately 10:00 p.m. local time. <br /><br />The problems began when the valve used to refuel deicing trucks froze due to cold weather. Deicing is essential for aircraft operations in cold weather since it helps remove ice, snow, and frost from the plane to accommodate a safe takeoff and flight.<br /><br />The airport attempted to continue operations after multiple flights were stuck in a line on the ramp. A plan to bring airstairs to remove passengers from the aircraft failed since the airstairs never arrived. Although one flight successfully disembarked, the towbar used to tow the plane broke afterward. Another issue was that low staffing for airlines operating at the terminal led to gates being backed up. Multiple other flights operated by American, United, and Southwest also faced long delays after arrival or at the gate before departure. <br /><br />The Federal Aviation Administration (FAA) later issued a ground stop for all flights at BNA due to poor weather conditions. The ground stop occurred from 4:00 p.m. to 5:15 p.m. local time on January 15. The Boeing 707 jetliner involved disintegrated mid-air shortly after departing from Tokyo, as a result of severe clear-air turbulence.</p><div><br /></div><div>The flight was a scheduled service from San Francisco (SFO/KSFO) to Hong Kong (HKG/VHHH) via Honolulu (HNL/PHNL) and Tokyo (HND/RJTT).</div>The Boeing 707 was expected to arrive at Tokyo Airport at 16:45 on the 4th of March. However, due to poor meteorological conditions at Tokyo and because the precision approach radar (PAR) of the GCA was out of service, it diverted to Fukuoka (FUK/RJFF) and landed there at 18:00.<br />After staying overnight at Fukuoka, Flight 911 left for Tokyo at 11:25 and landed there at 12:43.<br />The aircraft was prepared for the next leg to Hong Kong and a flight plan was filed for a flight. At 13:42 the crew contacted ATC requesting permission to start the engines and clearance for a VMC climb via Fuji-Rebel-Kushimoto. The aircraft left the ramp at 13:50. It was instructed to make "a right turn after " and departed Tokyo Airport at 13:58. After takeoff the aircraft flew over Gotemba City on a heading of approximately 298 deg at an altitude of approximately 4900 m and indicated airspeed of 320 to 370 knots. The aircraft, trailing white vapor, then suddenly lost altitude over the Takigahara area, and parts of the aircraft began to break away over Tsuchiyadai and Ichirimatsu. Finally, over Tarobo at an altitude of approx. 2000 m, the forward fuselage broke away. The cabin crew performed cardiopulmonary resuscitation (CPR) as the Boeing 737-800 diverted to Providenciales for an emergency landing. Despite the quick response and efforts of the flight crew for the unforeseen medical emergency, a landing was required so that further medical treatment could be provided.<br /><br />The plane landed in Providenciales at approximately 6:18 p.m. local time. Before the landing, the police received an emergency landing call at 6:12 p.m. and dispatched a medical team. To maximize the working space within the cargo hold, the cargo doors opened outwards, making them vulnerable to being forced open at high altitudes under normal in-flight pressure. To prevent this, a special latching system was used that locked shut under pressure when properly closed. To ensure the latches were properly positioned, a handle on the outside of the door pressed small metal pins into the latches; if the latches were not in the proper location the pins would not align and the handle would not close.<br /><br />On Sunday the 3rd of March 1974 flight TK981 departed Istanbul (IST/LTFM) for a flight to Paris-Orly Airport (ORY/LFPO), France to London-Heathrow Airport (LHR/EGLL), United Kingdom. The DC-10 landed at Paris-Orly at 11:02am and taxied to stand A2. There were 167 passengers on board, of whom 50 disembarked. The aircraft was refueled, and baggage was loaded onto the plane. The planned turnaround time of one hour was delayed by 30 minutes. An additional 216 passengers embarked. Most of the passengers were booked on this flight because of a strike at British Airways. The door of the aft cargo compartment on the left-hand side was closed at about 12:15 pm. When all preparations were complete the flight received permission to taxi to runway 08 at 12:24. Four minutes later the crew were cleared to line up for departure and were cleared for departure route 181 and an initial climb to flight level 40. The aircraft took off at approximately 12:30 and was cleared by Orly Departure to climb to 6000, which was reached at 12:34. The North Area Control Centre then cleared TK981 further to FL230. Three or four seconds before 12:40:00, the noise of decompression was heard, and the co-pilot said: "the fuselage has burst" and the pressurization aural warning sounded. This was caused by the opening and separation of the aft left-hand cargo door. The pressure difference in the cargo bay and passenger cabin, the floor above the cargo door partly collapsed. Two occupied tripe seat units were ejected from the aircraft. All the horizontal stabilizer and elevator control cables routed beneath the floor of the DC-10 and were thus also severely disrupted. Also, the no. 2 engine power was lost almost completely. The aircraft turned 9 deg to the left and pitched nose down. The nose-down attitude increased rapidly to -20 deg. Although the no. 1 and 3 engines were throttled back the speed increased to 360 kts. The pitch attitude then progressively increased to -4 degrees and the speed became steady at 430 kts (800 km/h). At a left bank of 17 degrees the DC-10 crashed into the forest of Ermenonville, 37 km NE of Paris killing the 11 crew and the 335 passenger - total of 346 SOB.<br /><br /><br />The crash is also known as the Ermenonville air disaster. At the time, Flight 981 was the deadliest plane crash in aviation history until 27th March 1977, when 583 people perished in the collision of two Boeing 747s in Tenerife. It remained the deadliest single-aircraft accident until the crash of Japan Airlines Flight 123 on the 12th of August 1985, and the deadliest aviation accident without survivors until the Charkhi Dadri mid-air collision on the 12th of November 1996. It remains the deadliest single-aircraft accident without survivors, the first fatal and deadliest crash involving the McDonnell Douglas DC-10, and the deadliest aviation accident to occur in France. The domestic terminal is closed from 11pm until 4am.<br /><br />Passengers who have missed connections or arrived extra early for morning flights have made many complaints about being kicked out and forced to wait on the street. Entrepreneur and Planet Ark co-founder Jon Dee said he had a 6am flight to Armenia via Dubai on Monday morning. He decided to get the last train from his Katoomba home on Sunday night and arrived at the airport just after midnight.<br /><br />He said hotels were booked out or charging inflated prices due to Taylor Swift performing in Sydney. He said he planned to relax in the airport until it was time to check in at 3am but was "amazed" to be told by a security guard to leave the premises.<br /><br />"When I went outside, I met this German lady who had a six-month-old baby, and she had to wait for two-and-a-half hours outside," Mr. Dee told ABC Radio Sydney.<br /><br />Like him, she had assumed there would be somewhere to wait even if their flight wasn't leaving until 6am. "I travel around the world, all the major international airports, the terminals themselves are open 24 hours a day even if they're not allowed to fly 24 hours," Mr Dee said.<br /><br />"That way, if you do arrive there, or if you're stuck, or the plane breaks down, there's some way you can get a coffee, somewhere you can go to the toilet, somewhere you can relax."<br /><br />Mr. Dee said it was "embarrassing".<br /><br />"We are the main entry point for most people coming to Australia and it's ridiculous that we're kicking people out on the street. "Imagine if it had been a winter's night with rain, all of those passengers would have got wet, I think it's time for it to become a 24-hour airport."<br /><br />A spokesperson for Sydney Airport said the terminals closed to the public overnight for security reasons. "It's due to the government-mandated curfew from 11pm to 6am," they said. "However, we emphasise a commonsense approach with our security contractor particularly around situations involving vulnerable people.<br /><br />"We will follow this up to make sure our teams meet expectations around customer service and communication in the future." Mary Loonam said she had a similar experience when she and her friend arrived early for their 6am flight to Eastern Europe.<br /><br />"I've got this friend who has to be early for everything, so we arrived at the airport by taxi and of course nothing was open," she said. "And she suffers from quite a bad back that friend of mine and there was no way for her to sit or anything.<br /><br />"She ended up sitting on a suitcase."<br /><br />Karen Burgess said she arrived at the airport at 2am to farewell her son who was travelling to the United Kingdom. "There were probably about 20 to 30 people there … I just thought it was hysterical, I had my nose pressed to the window looking at the guy polishing the floor."<br /><br />Aviation expert and Aviation Projects managing director, Keith Tonkin, said he was not surprised Sydney Airport was closed for a few hours given the flight curfew.<br /><br />He said there would be a "significant cost" to open the terminal facilities for a relatively small number of people who would need them at those times. "It is unlikely that they would have had any airline staff and other staff that would want to be there necessarily, during those times if there was no scheduled operation," Mr. Tonkin said.<br /><br />He suggested the airport consider providing some basic shelter to accommodate those people who were extra early for flights or have had their plans disrupted leaving them stranded. When United started flying into Brisbane from San Francisco 16 months ago (commenced 28th of October 2022), I said to my family and friends, Delta will be next. It took longer than expected, but it's happening. <br /><br />Delta Air Lines will join rivals American Airlines and United Airlines in flying between Brisbane and the USA. <div>As posted on my blog on the 3rd of February, American airlines will commence on the 27th of October 2024.<br /><br />The skies between Brisbane and the USA are about to become a little more crowded, but that's good news for Queenslanders, as Delta Air Lines reveals plans to fly between Brisbane and Los Angeles. Although only a seasonal service running between December 2024-March 2025, the flights will increase competition and put downward pressure on airfares.<br /><br />Delta will fly its Airbus A350 with private Delta One business class suites plus premium economy recliners on the 13h 30m route, with three flights each week from the 4th of December 2024 through to the 28th of March 2025.<br /><br />The moves see's the SkyTeam member competing directly against Qantas for Brisbane-LAX travellers, and with a far superior proposition when you compare the quiet, modern A350 and its premium seating against the much older A330 which Qantas has pressed into service trans-pacific service due to a shortfall of long-range jets.
